Each opening gets checked before anything is ordered. A sound frame gets an insert window set inside the existing opening. A frame with rot or real structural damage gets pulled down to the rough opening and replaced full frame instead, because setting a new window inside a compromised frame just seals the problem back up behind new trim.
Every opening finishes the same way regardless of which method it needed: sealed, insulated around the frame, and capped in aluminum on the exterior trim so the wood underneath stays covered and dry going forward.
